Empowering Indigenous telecare Needs
Community TeleCare is the empowering tool that Indigenous communities need to achieve parity in health access. It puts the power in their hands to live a life of opportunity, comfort, and respect, taking control of their health and wellbeing.
Through our unique approach, we utilize Community Health Needs and TeleCare Team Readiness Assessments to help communities identify and overcome healthcare barriers. Our expertise lies in facilitating the recruitment of providers who value and promote whole-person treatment for chronic diseases, mental health, and allied health services delivery through virtual care.
Our technology partners deeply respect and honour Indigenous history and culture. This respect is most evident in our Patient Intake process, which encourages patients to share their Life Story, not just their Health History. Through AI, Elders' Life Stories can be translated to Indigenous language, becoming powerful, documented journeys that inform physicians of
multiple impacts on health and wellbeing. This approach not only respects the community's culture but also provides a wealth of knowledge to build a culturally representative path to wellbeing, helping TeleMentor youth understand and preserve their community history.
